Is It Necessary to Perform Surgical Staging in Patients with Giant Immature Ovarian Teratomas Mimicking Mature Cystic Teratoma at Frozen Section

چکیده
Giant mature teratomas (>20 cm in diameters) sometimes may contain malignant immature elements or tissues. These small foci of immature tissues could be missed at frozen section resulting inadequate initial surgical intervention. There is no large series in the literature either showing the accuracy of frozen section on huge ovarian teratomas or concerning the adequate surgical approach at the initial surgery for giant immature teratomas diagnosed as mature cystic teratomas at frozen section in patients with elevated tumor markers. We presented a case with unilateral giant immature ovarian teratoma and peritoneal gliomatosis which diagnosed as mature cystic teratoma at frozen section. Although elevated tumor markers (CA-125, AFP, CA-19,9 CEA, and lactate dehydrogenase), ascites and peritoneal implants were suggested malignant disease, staging surgery was not performed due to the benign report of frozen section. Since the patients having peritoneal implants of mature glial tissue have an excellent prognosis regardless of the grade of the ovarian teratomas, it may be suggested that surgical staging at the initial operation may not be performed in patients with giant immature teratoma diagnosed as mature teratoma at frozen section despite to the elevated tumor markers, presence of ascites or mature peritoneal implants. seems that using it in infertile patients has beneficial effects. 1Gazi University School of Medicine, Department of Obstetrics & Gynecology, Ankara, Turkey. 2Gazi University School of Medicine, Department of Pathology, Ankara, Turkey. The diagnostic value of ultrasound for ovarian mature cystic teratoma and accordance of it with postoperative histopathologic findings
Background: Mature cystic teratoma of the ovary is one of the most common benign ovarian tumors, which may be confused with other adnexal lesions due to its extensive manifestations on ultrasound. Therefore, in this study, we tried to reduce the need for pathology frozen sections by examining more accurate preoperative ultrasound results and classifying ovarian lesions based on them. Methods: ...
متن کاملImmature Teratoma after Three Laparoscopic Resections for Mature Cystic Teratomas
We report a case in which an immature teratoma developed following three previous resections for mature cystic teratomas. The patient was a 26-year-old nulliparous woman with a regular menstrual cycle. Twelve years earlier, she had consulted a pediatrician for complaints of lower abdominal pain. Bilateral cystic teratomas were suspected and she underwent a left salpingo-oophorectomy and a right...

Teratomas are most common germ cell tumors with two or more germ cell layers. Mature teratomas are common compared to Immature teratomas. These tumors are common in young and adolescent girls and induce large impact on their reproductive life and on the natural life span, which in turn depends upon the complications in case of Mature teratoma; Grading and staging of tumor in Immature teratoma. ...
متن کاملGiant Mature Primary Retroperitoneal Teratoma in a Young Adult: Report of a Rare Case and Literature Review
Teratomas are neoplasms of the embryonic tissues that typically arise in the gonadal and sacrococcygeal regions of adults and children. Primary adult retroperitoneal teratomas are rare and demand challenging management options. We report a case of a unilateral primary retroperitoneal mature cystic teratoma mimicking an adrenal mass in a 28-year-old female patient. Complete resection of the mass...
متن کاملA Giant Mature Cystic Teratoma Mimicking a Pleural Effusion
The vast majority of teratomas originating from more than a single germ layer are benign. Often, such teratomas are initially asymptomatic. Later symptoms are caused by the weight per se of the teratoma and include chest pain, cough, dyspnea, and/or recurrent attacks of pneumonia. A mediastinal teratoma is treated by total surgical resection of the mass. Here, we report a case of giant mature c...
